It currently is creativefirstimpressions.com's policy not to collect personal information on any person under the age of 13. We use photos of families, babies and children for our birth announcements and other card designed by Creative First Impressions on our website and advertising. We never use the child's or the parent's correct name on any advertising. If we request to place you or your child's photo on our website or advertising, a model release form will need to be signed by the person if over 18 or by the legal guardian if under 18. A copy of our PDF Model Release Form is available for download. A signed copy of the Model Release Form will need to be mailed to Creative First Impressions, Inc., 260 Scarlet Blvd., Oldsmar, FL 34677, or faxed to 813.854.5981 to have your photo appear on our website or advertising.
